NOTICE OF PUBLIC

HEARING

The City of Lockhart Planning and Zoning Commission will hold a Public Hearing on Wednesday, June 22, 2011, at 7:00 P.M. in the lower level of City Hall, 308 West San Antonio Street, Lockhart, Texas, to receive public input regarding the following:

SUP-11-08.  A request by Jesus Lujan for a Specific Use Permit to allow conversion of an existing large storage building to a fellowship hall as an expansion of a Church as a Public/Institutional use in the CMB Commercial Medium Business District, on 2.43 acres in the Byrd Lockhart League, Abstract No. 17, located at 401 North Commerce Street.

SUP-11-09.  A request by Carlos Martinez, Jr. for a Specific Use Permit to allow a Manufactured Home on 0.33 acres in the Byrd Lockhart League, Abstract

No. 17, zoned RHD Residential Heavy Density District and located at 328 Pine Street.

MEDICAID PROVIDER AGREEMENT

On June 01, 2011, the Medicaid agreement between Century House and the Texas Department of Aging and Disability Services relating to the ICF/MR facility”s participation in the Title XIX Medicaid Program is to be terminated, unless the facility achieves compliance with the Medicaid requirements prior to that date. The cause for Medicaid termination is that Century House does not meet minimum health or safety standard required for Medicaid program participation. The Medicaid program will not make payments for ICF/MR services furnished to residents of Century House, on or after June 01, 2011.
