Lockhart to hold athletic physicals

The Lockhart Athletic Department will be offering low-cost athletic screenings and physicals for the students-athletic, band, cheer and Lionettes of Lockhart ISD.
Physicals will be offered in the front entrance of Lockhart High School on Wednesday, May 9. High school and junior high school girls’ physicals will begin at 5:15 p.m. and high school/junior high school boys will start at 6:15 p.m.
Cost for the physical is $20.
All UIL & LISD required forms must be completed before a student-athlete participates in any practices, games or matches before, during or after school, (both in-season and out-of- season). Cheer, dance, and band must have a yearly physical on file.
Online forms were available beginning Monday, May 1.
Hard copies of the Physical and Medical history forms will be passed out the week of May 1 along with directions on how to complete the online paperwork.
Computers will be available on May 9 to help assist parents with online paperwork.
Online paperwork for athletes only must be completed before May 9 or on site the dayof physicals.
Officials urge you to please have your Medical History form filled out when you arrive for athletic screening and physicals.
